How do I get my loved one admitted?
- If your loved one is in the hospital, have their Case Manager send a referral to Big Horn Rehab at 307-204-1510
- If your loved one is at home and needs placement, have their Primary Care Doctor send a referral with progress notes, medications and allergies to Big Horn Rehab at 307-204-1510
Does insurance pay for their stay?
- If the patient is coming in from the hospital for Short Term rehab, they must have a 3 night stay. Medicare will pay fully for their first 20 days. Every secondary insurance has different coverage plans and we would let you know what your secondary insurance would cover.
- Most insurance plans do not cover Long Term Care, unless they have a nursing facility plan. This means the resident would be private pay until they would qualify for Wyoming Medicaid. We do assist in the Wyoming Medicaid application process.
- VA – We are VA contracted. If the resident has VA benefits, prior authorization through the VA would be required.
What is the difference between Respite, Memory Care, Short Term and Long Term?
- Respite – We would admit your loved one for a specific amount of days, this is private pay
- Memory Care – We have a secured unit for residents who may wander or need more attention, most with Dementia/Alzheimers diagnosis. It is not a behavioral unit
- Short Term – Your loved one would be admitted to get therapy, whether physical, occupational or speech after orthopedic surgery, a stroke or failure to thrive. These residents generally discharge home within 100 days of admittance.
- Long Term – Your loved one needs 24-hour nursing care, 365 days of the week.
